Patient Forms

Discover how to add a patient form to the client record.

A new feature we have added is the ability for the patient to receive and fill in different forms such as registration and clinical forms. It also will allow you to generate and issue surveys out to patients.

You can action this via the 'New Form' option at the top of the patient record:

These forms can be created and edited through the settings. You will see a tab in the settings, called 'Forms', simply click on this tab. There is a useful page on setting up and customising forms, click here to view more information.

Selecting the New Form option at the top of the patient record will let you view your two options of different types of forms you may have created, Registration and survey.

Selecting either option here will then display the forms you have made in the respective section:

once you select the form you want to action with this patient you have three options:
send to client, in person client entry and fill in on behalf of client.

Send to client sends the form out to the patient.

In person client entry will prompt you to enter a pin so you can then pass over the device to the patient to fill out in your presence - this means they cant go anywhere else on the system until the form is complete.

Fill in on behalf of the client allows you to fill in the form yourself.

If sent out, The form will be shown on the patient's record, when they fill out the form and submit the form back the status will change to 'Submitted'. At this stage you have the ability to view and review the document and approve it to change the status to completed:
